On the test report (not the certificate) I have from Optigen there is a blurb at the bottom that states to make a copy and submit it with the applicable fee to OFA and then a space for signature of the owner.
Is this not sufficient? Just wanting to make sure I get it right the first time J

Julie -- here is the l ink to the Applications section of OFA -- what you want is the Application for Genetic DNA Database. It takes 2-3 weeks after you send it in to appear on the OFA site -- you should get your OFA certificate in the mail at about the same time.
Be sure to fill in the space for the microchip or tatoo number.
