No, the "stickers" that Leita has sent to you are ON the application. Its one 8 1/2 x 11 sheet of paper with the sticker already placed at the bottom. You fill in your own information on the application when you receive the sheet from Leita, attach a copy of your CD certificate and mail the whole two pages to OFA. *LahrGSP@aol.com* wrote: *OK so I need to get the application?
