I believe you also have to send in the OFA Application for the Genetic Database which can be downloaded from the OFA web site. It's on this Application, at the bottom where it says Fees, that you put the GSPCA CD sticker. It is also on this Application that you list the microchip or tatoo number -- without permanent identification, you dog cannot be eligible for CHIC even though it may have all the clearances.
